Episode Management

BlackTree’s episode management model takes a multi-phase approach, beginning with an onsite analysis and plan development with off-site monitoring and support for an extended period of time.

During the onsite analysis, BlackTree will assess specific areas of operations, including, but not limited to the intake and referral process, authorizations, clinical case management, care plan development, disease management, therapy utilization, staffing and productivity, discharge planning and software system utilization.

BlackTree will review each of these areas and work with your agency’s clinical management team to design the processes and implement the tools needed to provide data-driven, proactive management of your episodic care. Ongoing BlackTree management of the project will help your team to remain focused and engaged in the ongoing plan, enabling you to realize the maximum potential of improved clinical and financial outcomes.
